Patients of Rosewood Medical Center
ICU Department, Sixth Floor
Rosewood Medical Center

Dear Patients,

My deepest condolences go out to you. I am informing you of an unfortunate accident that has occurred recently.

The initial blood samples that you gave us are no longer valid. On February 13, 2008; there was an accident in which the blood samples we obtained spilled over. The samples were destroyed and therefore are no longer accessible. The mishap was a result of poor procedure in which a technician left the sample cart unattended, and the cart tipped over spilling the samples on the floor. The hazardous team has cleaned up the accident. We can guarantee you are safe from any hazardous materials, and there is no need to worry about the cleanliness of the hospital.

We have reviewed the ones at fault, and are currently in the process of disciplinary action. We apologize for this inconvenience and promise this is an isolated incident. To insure this does not happen again, we have prepared new safety procedures. These new safety procedures are extremely detailed and will allow the blood samples to travel from lab to lab without any disruption. As well as this, the technicians are being trained extensively to handle the blood samples on the cart with extreme care. We have designed new routes for the blood samples to be transported on. These new routes will avoid cracks, bumps, and areas with high traffic levels in the hospital. This was a very unfortunate event, and we want to be sure that it does not happen again.

Since all of the blood samples have been destroyed in the accident, I regret to inform you that we will need to draw a new blood sample from you. I am tremendously regretful for this situation. The last thing we want to do here at Rosewood Medical Center is inconvenience any of our patients. Since this incident is, one hundred percent by fault by the staff, I can assure you that these new samples will not be added to your hospital bill. They will simply replace the old samples and there will be no added costs.

The new samples will be taken at a time that is best with you. I am asking that you clarify with your allotted nurse, and or doctor, a time in which you will be the most available for this blood drawing. The drawings will need to be taken in the next week, to ensure that all patients are given their blood readings in a reasonable time. So it is very important that you talk to your nurse or doctor as soon as can.

Our hospital values all of its patients and their feedback. If you have any questions, comments, or problems; please feel free to contact me whenever you want. You can reach me by phone at (339) 555-8979, ext. 541, or you may e-mail me at georgej@rosewoodmed.org. Again I am extremely sorry for this inconvenience, and I thank you for taking the time to read this important but unfortunate letter.

Sincerely,

George Jones
